Fill it out, and write the address on the application where you would like to receive your actual Ballot. You can drop this form off at the Drop Box located outside Town Hall OR mail it back right away (don’t forget the stamp on the envelope).
- Request an Application from Town Clerk’s office at the Town Hall. You can do this by calling (860) 675-2380, emailing frinkm@farmington-ct.org (the Application will be mailed to you), or by stopping by the office in person (8:30am-4:30pm Mon-Fri) and filling out the Application there.
